Waiver of Liability and Hold Harmless Agreement:
I understand that playing pickleball presents a risk of injury. I hereby assume all risks of any injury that I may suffer, as a result of participating in any pickleball activity sponsored by the Florence Oregon Pickleball Club, and any activity whatsoever occurring on the premises of any pickleball facility that is in any way maintained and/or use permitted by that club/association or supervised by it. I hereby waive and release all claims or causes of action that I may acquire against the Florence Oregon Pickleball Club, the City of Florence, their officers, members, representatives, and agents from all liability to me resulting from any injury I may suffer as a consequence of any alleged negligence of any such entities or persons in connection with the sponsorship and conduct of any such event or the maintenance or supervision of any pickleball activity.
Hold Harmless: I further agree to indemnify and hold harmless any venue, any other organization and/or organizers assisting in the previously mentioned event of the Florence Oregon Pickleball Club from any claims, suits, costs, expenses, damages, and liabilities, including attorney’s fees brought as a result of my involvement or participation in any previously mentioned event of the Florence Oregon Pickleball Club.

Use Permission: I also give the Florence Oregon Pickleball Club, its agents, or designees permission to use or distribute, without limitations or obligations, my image, name, voice, and words for any purpose connected with any event of the Florence Oregon Pickleball Club participates in. This will also include promotional and/or marketing materials.
This waiver of liability shall remain in effect so long as, and whenever I engage in any activities described herein.

Your consent below indicates your signature and you have read and understand the waiver of liability. All members will follow the Code of Conduct of the USAPA and follow the rules of conduct governing membership. Each member must give consent. Membership is subject to Board approval.
